Convenient, accurate, state-of-the-art lab testing is done to verify the pregnancy and to check the blood count and Rh blood type. The dose of methotrexate is calculated by a formula using the patient's height and weight. She then uses misoprostol tablets 2-7 days later, after which she can expect to experience lower abdominal cramping pain, vaginal bleeding, nausea, vomiting, and diarrhea, which may be extreme in some cases. It is absolutely necessary for the patient to return to this office two weeks after the initial medication for a follow-up exam and ultrasound to ensure the successful completion of the procedure, and afterwards as necessary.

Extensive studies have shown that approximately 5% of the time, non-surgical abortions are not successful (compared with less than 0.1% for surgical abortion), and the abortion must be completed with a surgical procedure called a suction D&C (dilatation and curettage). Occasionally, some patients will experience prolonged and heavy bleeding, of more than two sanitary pads per hour for at least two hours, even after the two-week follow-up visit, in which case she should call our office immediately to come in to be evaluated, and a suction D&C (dilatation and curettage) may be necessary. Failure to have the suction D&C can lead to complications, such as continuous, profuse blood loss that may require a blood transfusion, or a continuing pregnancy with birth defects.

Since non-surgical abortions are performed for early pregnancies, some very early pregnancies will turn out to be ectopic pregnancies (pregnancies located outside the uterus usually in the fallopian tube), which account for 2% of all pregnancies and can be life-threatening emergencies. Ectopic pregnancies are often difficult to detect. On September 28, 2000 the Food and Drug Administration announced the approval of RU 486, the controversial drug which is often referred to as the "abortion pill." RU 486 has long been the topic of heated debate among pro-lifers, pro-choicers, and politicians. Like many Americans, Presidential candidates Al Gore and George Bush have conflicting views of the abortion pill, with Gore supporting approval and Bush opposing approval of the abortion pill.
An estimated 500,000 women in Europe have used RU 486 since 1988. Today's approval of mifepristone, which will be marketed under the brand name Mifeprex, provides women in the United States with a pharmaceutical option for ending unplanned pregnancy.

The drug is expected to be available in physician's offices in about one month. However, physicians who prescribe the mifepristone must be willing to perform a surgical abortion should the drugs fail to terminate the pregnancy.

How Is RU 486 Used?
According to rules set by the FDA, the use of RU 486 requires three visits to a physician's office. RU 486 can only be prescribed by a physician after a HCG blood test, and a sonogram confirm and date the pregnancy. RU 486 is approved for use up to the 49th day of pregnancy, or about 7 weeks.
Once pregnancy is confirmed a woman returns to her physician's office for 2 doses of medication. First, she is given one dose of mifepristone, a synthetic steroid, which causes a fertilized egg to be unable to remain attached to the lining of the uterus. The second pill misoprostol, which causes uterine contractions, is taken 2 days later and terminates pregnancy anywhere from 6 hours to one week later. Both pills are taken in the physician's office.

Later a woman must return to her physician for confirmation that the pregnancy was terminated. If the pregnancy is still present a surgical abortion must be performed.

Possible side effects of RU 486 include uterine cramps, fatigue, nausea, and heavy bleeding. Most women will recover completely within a few hours or a week. The cost of the abortion pill is comparable to the cost of surgical abortion procedures at $300 to $500. IMPORTANT SAFETY INFORMATION
Although cramping and bleeding are an expected part of ending a pregnancy, rarely, serious and potentially life-threatening bleeding, infections, or other problems can occur following a miscarriage, surgical abortion, medical abortion, or childbirth. Prompt medical attention is needed in these circumstances. Serious infection has resulted in death in a very small number of cases in which misoprostol was used in the vagina. There is no information that vaginal use of misoprostol caused these deaths. If you have any questions, concerns, or problems, or if you are worried about any side effects or symptoms, you should contact your provider.

Be sure to contact your provider promptly if you have any of the following:

Heavy Bleeding. Contact your provider right away if you bleed enough to soak through two thick full-size sanitary pads per hour for two consecutive hours or if you are concerned about heavy bleeding. In about 1 out of 100 women, bleeding can be so heavy that it requires a surgical procedure (surgical abortion/D&C) to stop it.

Abdominal Pain or “Feeling Sick”. If you have abdominal pain or discomfort, or you are “feeling sick”, including weakness, nausea, vomiting, or diarrhea, with or without fever, more than 24 hours after taking misoprostol, you should contact your provider without delay. These symptoms may be a sign of a serious infection or another problem (including an ectopic pregnancy, a pregnancy outside the womb).

Fever. In the days after treatment, if you have a fever of 100.4°F or higher that lasts for more than 4 hours, you should contact your provider right away. Fever may be a symptom of a serious infection or another problem (including an ectopic pregnancy).

Take your MEDICATION GUIDE with you. When you visit an emergency room or a provider who did not give you your Mifeprex, you should give them your MEDICATION GUIDE so that they understand that you are having a medical abortion with Mifeprex.


The success rate for RU 486 is 92 to 92.5% during the first seven weeks of pregnancy. However a small percent of women who take RU 486 will require blood or additional surgery.

A woman who suspects she may be pregnant with an unplanned pregnancy and who wants to use the abortion pill to terminate her pregnancy should contact her physician or an area clinic as soon as possible to discuss her options.
